LYMPHOCYTOSIS AFTER SPLENECTOMY IN CHRONIC IMMUNE THROMBOCYTOPENIC PURPURA CASES

Spleen is an organ, playing an important role in lymphocyte traffic, and an some of the splenectomy applied patients lymphocytosis arise. Though this raise is proposed to be a reactive one, there have been such cases in literature showing chronic lymphocytic leukemia after splenectomy. A total of 17 cases, 3 men and 14 women with an average of 43.3 years of age, on which splenectomy is applied with chronic immune thrombocytopenic purpura diagnosis have been studied. Cases having lymphocyte level 4000/µL and a base are supposed to be lymphocytosis. 6 of 17 cases (35.2%) developed lymphocytosis. These had lymphocyte level of 5751.6 ± 1070.5/µL whereas the after II cases 2809.0 ± 669.4/µL (p<0.001). Examining lymphocyte subgroups, no difference has been observed between two groups due to percentage values. When we compare the absolute values; it has been found out that, the absolute B lymphocyte amount is 977.0 ± 510.0/µL in lymphocytosis group and 353.6 ± 258.2/µL (p<0.05) in the second group, the absolute T helper (T4) lymphocyte level is 1846,0 ± 357.3 /µL in lymphocytosis group and 782.2 ± 346.6/µL (p<0.001) in the second group, absolute T suppressor (T8) lymphocyte amount is 1750.5 ± 837.2/µL in lymphocytosis cases and 575.4 ± 220.3/µL in the second group (p<0.05). Absolute natural killer (NK) cell level has occured to be 540.1 ± 444.4 /µL in lymphocytosis group and 316±139.6/µL in the second group. On the after hand no statistical difference has been detected between these values (p>0.05). Consequently, on some of the splenectomy applied cases lymphocytosis have arousen and this raise affect the lymphocyte subgroups, being more in B lymphocyte, T helper lymphocyte, T supressor lymphocyte but no significant change of level in NK cells.
